    • First Physicians Group does not accept “Short Term” or “Limited Benefits” Plans administered by United Healthcare because these plans do not meet the minimum essential coverage requirements as stated by the Affordable Care Act (ACA). These plans are not regulated by the ACA and are commonly referred to as Limited Benefit Plans and Discount Plans. These types of plans are often identified on the member’s insurance card using names like, “ALL PremierChoice,” “Premier MED,” “Short Term Medical,” or “TriTerm Medical. This may include policies/plans marketed as Freedom Life, US Health Group, or Golden Rule.

* Pending provider credentialing: These are new Agreements. Not all of our physicians are finished with credentialing at this time. Please check the insurance provider’s website to see if the physician you want to see is listed as “in-network” before scheduling an appointment.

First Physicians Group may not be contracted with every product for every health insurer listed, nor at every FPG office location. Please call your health plan for specific information.

Your rights and protections against surprise medical bills – When you get emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing. For more information, click here.

Under the law, patients who do not have insurance or are not using insurance have the right to receive a “Good Faith Estimate” detailing the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment and hospital fees. For more information, click here.
